Home Health Services:
At United Geriatric Care Management Services we refer licensed, trained, and experienced aides in the home. Our goal is to keep our clients independant while remaining safe and secure in a comfortable and familiar home environment. Our Geriatric Care Management Division oversees our home care division, ensuring that the necessary and appropriate level of care is being administered to each and every home care client.
Home Safety Evaluation:
The staff at United Geriatric Care Management Services is proficient at assessing and providing suggestions to secure clients safety in areas of their home where they are most prone to falls. This includes grab bars in the bathroom, medical alert response system, etc. Social Recreational Activity Planning.
The staff at United Geriatric Care Management Services will establish a specific care plan which will minimize the risk of social isolation while ensuring continued cognitive development. Through the local Area on Aging, local community centers and participation with the local Alzheimer's Association, United Geriatric Care Management Services can facilitate a client to select the appropriate day care activity setting.
Arrange in Home Care:
Arrange start of care for home aides/companions. Facilitate interviewing process for in home private duty. We are experts at maximize a client's availability to obtain full reimbursement from their Long Term Care insurance. We take all long term care insurances.
Guardianship:
Executive director Joseph Bensmihen, MSW, GCM, is authorized to serve as Guardian for Palm Beach and Broward Counties. This service can facilitate when families are not able to obtain Durable Power of Attorney.
Monitoring Programs:
The staff at United Geriatric Care Management Services serves as a surrogate son or daughter to family members living in Florida. Visits can be conducted on a daily, weekly or monthly basis. This provides peace of mind to the family member who is out of state with the reassurance that staff is available 24 hours/day, 7days/week. This program allows the client to maximize their independence while maintaining a secure and safe home environment.
Medicaid Planning:
Staff at United Geriatric Care Management Services evaluates and assess client and/or family eligibility for State funds. Facilitation with an attorney who specializes in eldercare law will be included to legally secure the financial future of the client.
Alternative Living Arrangements:
Staff at United Geriatric Care Management Services facilitates and coordinates this decision to a smooth process of relocating a family member from the home environment to a retirement, assisted living community or skilled nursing facility. Through our local connections and relationships, this process is facilitated professionally, smoothly and securely.
Compliance with Physician Appointments/Medication Orders:
Through the office staff at United Geriatric Care Management Services, we coordinate with the client's health care professionals (i.e., physicians) all initial and follow up appointments. The geriatric care manager will also attend physician appointments with the client. Further, family members are informed of the current status of the client's medical condition.
